Life isn't simple any more. With managed health care we need to collect data from our patients before they are seen. There are also forms that our doctors utilize to review and patient information, and may need to be completed prior to your office visit. In order to expedite your visit, below are several forms that may be required upon visiting one of our offices. We strongly suggest that you download and complete them at home prior to your visit. This will significanly decrease your wait time.

Patient Registration Form - **Complete and bring in for your child's first visit and annually thereafter.
Childs History Sheet - **Complete and bring in for your child's first visit.
HIPPA Notice of Privacy Practices - **Complete and bring in for your child's first visit.
Consent to Treatment/HIPAA Acknowledgement - **Complete and bring this form with you during your child's first visit.
